Well North
Well North is a pilot anticipatory care project to improve the health of people experiencing health inequalities in remote and rural areas in the North of Scotland. Well North exists as six projects in remote and rural areas in the North of Scotland involving five health boards:
The project particularly focuses on early intervention with adults at higher risk of coronary heart disease and diabetes. It aims to increase the rate of health improvement in remote and rural communities by enhancing primary care or community services to deliver anticipatory care.
Well North will do this by:
- testing approaches to address the relationship between rural social deprivation and health inequalities
- identifying and targeting those at particular risk of preventable serious ill-health (including those with undetected chronic disease)
- offering appropriate interventions and services to them.
NHS Grampian
- Has a practice in Dufftown operating a self caring community initiative focusing on specific chronic diseases and conditions. This initiative has also now been extended to the neighbouring Rothes area.

- NHS Grampian are also piloting the use of Adult Healthy Weight Care Pathways with a view to identifying patients who are overweight or obese with other disorders or diseases. They are looking to work with a cluster of rural general practices to provide evidence-based lifestyle interventions and support existing staff to improve access to a healthy diet and physical activity within these communities.

NHS Highland
- Reviewing existing services in North West Sutherland with a view to identifying people at risk and then looking to work with them toward increasing access, provision and timing of services to support their needs.
- Well North Highland (North West Sutherland) Programme contact: Christian Goskirk

NHS Orkney
- Is focusing on delivering inequalities targeted cardiovascular health checks for individuals aged 45-64.
- Well North Orkney Programme contact: Christina Bichan
NHS Shetland
- Is trialing a holistic approach using a deprivation and social exclusion tool to work with vulnerable and excluded individuals or households on Unst and Fair Isle. This approach has also recently been extended to three suburb areas of Lerwick, which had been identified as showing the highest levels of deprivation in Shetland.

NHS Western Isles
- Is seeking to implement "Better Health, Better Care" by enhancing the provision of anticipatory care through extending the screening and treatment service currently provided by their local multifunctional mobile health unit into Cardio Vascular Disease prevention and Diabetes screening.
